Terms and Conditions

Cancellation Policy: Cancellations made within 48 of your scheduled appointment may be subject to a $75 fee.

Late Policy: Please arrive on time for your appointment. Arriving more than 10 minutes late may result in a shortened service or may necessitate rescheduling your appointment with a $75 fee.

No Show Policy: If you do not show up for your appointment, a no-show fee of $75 will be applied to your card on file.

Visit Length: All appointments will end at the scheduled time so that we may serve the next patient at their scheduled start time. Please make sure you book an appointment that fits your needs.

Fees: We do not provide direct billing, however you will be provided with an invoice which can be submitted to your insurance for consideration. Fees will be deducted from your credit card on file after your visit unless another form of payment is preferred. In the case of forms, or procedures where an additional cost may be incurred. You will be informed in advanced of any additional charges.

Refund Policy: Our clinic does not offer any refunds for services purchased at The Care Collective.

Zero Tolerance Policy: We have zero-tolerance for any verbal or physical abuse (disruptive behaviour, inappropriate actions or language etc.). If behaviour is judged to be aggressive or intimidating in person, over the phone or by correspondence you will be discharged from the clinic and advised to access care at any available walk-in-clinic.

Controlled Substances: Occasionally controlled substances are necessary for treatment. Please note these medications will never be prescribed for longer than 30 days at a time. Renewals deemed necessary are subject to fees. You will also be asked to sign a controlled substance agreement, this is a practice standard.

Home Visits: Home visit requests are based on provider approval. You must be deemed in our catchment area. You will be asked to secure any pets. If the provider does not feel safe on arrival to your home you appointment may be cancelled without notice.
