Meet The Care Collective

The Care Collective is a fee-for-service clinic in Leslieville, Toronto run by three Primary Care Nurse Practitioners.

We opened our clinic not only to meet the growing need for access to care in the East End of Toronto, but also so we could deliver care in a way that makes us proud.

Our goal is to ensure you feel educated and empowered to partner in managing your own health.

We are committed to:

Collaboration

Communication

Professionalism

Patient Empowerment
