We are located at 20 Leslie St. Toronto, Ontario M4M 3L4
Our office is located on the second floor within Suite House Leslieville Unit 206. Please use your access code, found in your appointment confirmation email, to buzz in the exterior door and The Suite House. You can find us in Suite 30 or relax in the waiting room to the left of the main door until your appointment time.
Please note that our office is not readily accessible, we apologize for the inconvenience. Please contact us if you are still interested in our services, we can offer home visits to community members with mobility concerns.

We are hoping the provincial government will move toward funding Nurse Practitioners in Ontario so patients will be able to access NP care through OHIP.
