Well Child
Book: 40 Minute Consultation
​This appointment is for babies, children and adolescents. This visit will focus on growth and development and provide pre-emptive guidance for parents related to age and stage.
​
Visits should occur at 2m, 4m, 6m, 9m, 12m, 18m, 2 years and then annually. This is in line with the immunization schedule.

This visit includes:

History and physical assessment
-
Growth assessment using WHO growth charts
-
Developmental assessment using the Rourke and Greig tools
-
Autism Screening at 18 months.
-
Vaccinations
-
Ordering of any tests or imaging
-
Treatment plan for any identified concerns
-
Referrals Included.

Any results will be provided through our secure portal

Older Adult
Book: Comprehensive Health Visit
This is a comprehensive visit for older adults over age 70. Our aim is to help you age well and comfortably. We address any immediate concerns and create a plan with recommendations and resources.
This visit includes:

Detailed health history
-
Review of client's concerns
-
Medication review and adherence assessment
-
Risk factor assessment
-
Screening for geriatric syndromes
-
Physical assessment
-
Cognition screening and functional assessment (ADLS + IADLS)
-
Mental health screening
-
Any test or referrals deemed necessary

Medications will be reviewed using the Beer's Criteria with a goal of minimizing polypharmacy to keep interactions minimal.
​
We can also provide guidance on how to approach organizing POA, DNR paperwork so your wishes are understood by your family. We can help make referrals for alternate levels of care, community agencies for help at home and MAID referrals.
A summary of your visit will be emailed to you once we have the results from all your tests.

Women's Health
Book: 40 Minute Consultation
A women’s health visit is a vital part of your overall wellness. It’s a time to check in on reproductive health, screen for potential concerns, and talk openly about your body, lifestyle, and goals. ​

Follow up with results and any change in treatment plan will occur through the secure patient portal.
