At The Care Collective, we want to give you options.  We offer two pricing models.  Annual memberships and a more casual pay-as-you-go option.  

Annual Membership

Our Annual Membership secures your access to our  NP providers.  You will be one of only 500 rostered patients.

Our membership includes:

  • One Annual Comprehensive Visit (60 minutes)

  • Three 40-minute Visits 

  • Three 20-minute Episodic Visits 

  • Valued at $1300+

​Plus:

Same-day or next-day booking

Sick Notes and Insurance Notes (one page)

Does not cover procedures

No refunds at the end of the year.

Episodic Care

$Pay-As-You-Go

This model offers flexibility to individuals looking for episodic care only.

 

Visits are based on length of time needed:

  • 20 Minute Appointment $100 

  • 40 Minute Appointment $180

  • Annual Physical $350
